Providers

Our providers see all ages of patients with different levels of medical complexity. Our scope includes referrals women’s health, pediatric visits, procedures such as skin biopsies and knee injections. Please visit the services tab – you may be surprised by how much we can do!

The Team

Candace Percifull, CNP

​Candace is a board certified Family Nurse Practitioner, who received her degree from Arkansas State. She has lived in Albuquerque for 10 years with her family and two dogs, Duke and Lady. Candace attends Isotopes games as much as she can and plays for a local bowling league. Her number 1 priority is to make sure her patients are getting the best care that she can possibly provide while making it a relaxed and easy environment for everyone involved.

Amy Jane Lozoya, FNP-C

Amy is a board certified nurse practitioner. She has been a nurse since 2008 and obtained her masters of science in nursing in 2019 from Regis College. Amy strives to provide individualized care to each of her patients. Away from the office, Amy enjoys traveling and spending time with her family and friends.

Shauna Montoya, FNP-BC

​Shauna Montoya has 19 years of nursing experience and now enjoys the challenge of caring for her patients as a Certified Family Nurse Practitioner. Mrs. Montoya completed her Master of Science in Nursing degree from Chamberlain University. Shauna is compassionate about educating her patients regarding their health and developing goals alongside her patients to improve their quality of life. She is married and has 3 grown children, a granddaughter, and a fur baby, Raven. She loves Bible study, jogging/biking long distances, and walking daily with her husband.

Krystel Habish, FNP-C

​There is nothing better than watching a beautiful sunrise or sunset in New Mexico. With that being said, I am proud to say that I was born and raised in New Mexico. I am a board-certified Family Nurse Practitioner. I completed my Master of Science in Nursing at the University of Texas at Arlington. In addition, I hold a Bachelor of Science in Biology and Psychology. Prior to beginning my career as a Family Nurse Practitioner, my nursing background consisted of Infusion Nursing and Surgical Specialties. I am dedicated to leading exceptional patient care, advocacy, and quality outcomes in a compassionate manner. I am passionate about patient-centered care and shared decision making. My goal is to create an open space for patients to feel comfortable to discuss their medical concerns. Outside of work I enjoy spending time with my husband and three beautiful children. I also enjoy baking, getting lost in a book, traveling, and finding new shows to watch.

Jordan Pelock, PA-C

​I was born and raised here in Albuquerque, enjoying the seasons and the sunshine. I attended the University of New Mexico for my undergraduate degree in Exercise Science. I spent one semester studying abroad in Wales at the University of Swansea and then continued my education at UNM School of Medicine. I have a special place in my heart for Women’s Health and Family Medicine where I can give back and serve my community. Outside of work, I enjoy heading to the Isotopes, traveling the world, reading and snowboarding (when we get enough snow
